Advanced Practitioner

Monogram Health is seeking a skilled Advanced Practitioner to join our team of healthcare professionals. As a key member of our integrated Care Team, you will play a vital role in delivering high-quality, patient-centered care to individuals with chronic kidney disease and end-stage renal disease.
Key Responsibilities- Conduct comprehensive annual wellness exams and provide personalized care to patients in their homes and virtually.
- Counsel and educate patients and families about benefits and programs available to improve their health and well-being.
- Document patient information, including medical history, medications, and treatment plans.
- Coordinate care with primary care providers, specialists, and ancillary services to ensure seamless patient care.
- Prescribe medications, order tests, and collaborate with patients' Monogram physicians to deliver evidence-based care.
- Facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing.
- Participate in integrated care team meetings to ensure effective communication and collaboration.
- Bilingual (English/Spanish) highly preferred.
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work autonomously and practice without direct supervision.
- Access to transportation, a valid driver's license, and car insurance.
- Proficiency with medical instruments and equipment required by the work.
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology.
- Ability to communicate effectively with patients, families, physicians, and community representatives.
- Sound understanding of federal and state regulations, including HIPAA and OSHA.
- 2 or more years of direct patient care experience.
- Managed Care/IPA/Health Plan experience.
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
