Our Ideal Candidate:
We are seeking a skilled Social Worker (MSW) to join our team. The ideal candidate will have a Master's Degree in Social Work from a CSWE accredited School, licensure as required by the state, and at least one year of experience as a Social Worker in a healthcare and/or hospice setting.
Responsibilities:
- Provide psychosocial support to patients and their families based on an assessment of their needs and goals.
- Develop and implement a plan of care that addresses the patient's physical, emotional, and spiritual needs.
- Collaborate with the interdisciplinary team to ensure comprehensive care.
- Monitor the patient's progress and adjust the plan of care as needed.
What We Offer:

Salary Range: $87,000.00-$110,000.00
