MDS Coordinator RN

Henry Rehab and Nursing is seeking a skilled MDS Coordinator RN to join our team in a full-time, salaried position at our skilled nursing facility.

• Ensure accurate and comprehensive clinical assessments are completed in a timely manner.
• Collect and analyze data to optimize facility reimbursements.
• Collaborate with staff and residents to gather information and complete paperwork.
• Support the management team in maintaining a high level of care and service.

• Active state RN license.
• Understanding of the MDS process, including coordination, scheduling, and completion.
• Experience in SNF/long-term care.
• This is a direct hire position; we are not an agency.
