About the Role

We are seeking a highly skilled and dedicated Community Health Worker Supervisor to join our team at the South Side Healthy Community Organization Inc. This is an exciting opportunity to make a meaningful impact in the lives of individuals and communities.

Key Responsibilities
  • Supervise and mentor Community Health Workers (CHWs) in delivering high-quality health education and outreach services to community members.
  • Collaborate with project leaders and team members to assess intervention effectiveness, monitor CHW caseloads, and discuss workloads.
  • Participate in the hiring process for CHWs, conduct regular evaluations, and help CHWs establish professional boundaries.
  • Foster a positive teamwork environment, recognizing staff achievements and building strong relationships.
  • Ensure CHWs are well-equipped to succeed by managing workloads, documenting efforts, and overseeing program-specific paperwork.
  • Help CHWs develop business skills such as writing, public speaking, record keeping, leadership, and time management.
  • Facilitate effective communication between team members, project staff, community partners, and clients.
  • Assist with organizing data for program evaluation and ensuring the fidelity of data collected by CHWs.
  • Actively participate in team meetings and/or external partner meetings.
Requirements
  • Flexible work schedule required, with occasional evenings and weekends as needed.
  • Travel throughout the Chicagoland area, with a valid driver's license, reliable vehicle, and proof of insurance.
  • At least three years of professional experience in a public health, social service, or healthcare industry, complemented with a bachelor's degree or equivalent combination of experience and education.
  • Up-to-date vaccinations as defined by the CDC, including influenza and COVID-19, required prior to the start date.
  • CHW Certification preferred.
  • Strong proficiency with an EMR system and working knowledge of Microsoft Office Suite.
  • Bilingualism in Spanish a plus, and may be required for some positions.
