Rehabilitation Social Worker II

As a Rehabilitation Social Worker II, you will play a vital role in our team's efforts to create positive outcomes for our clients. Your expertise and compassion will make a significant difference in the lives of those we serve.

Key Responsibilities:

  • Conduct thorough psychosocial assessments with individuals and families to identify their needs and goals.
  • Lead the interdisciplinary team in assessing the family's ability to understand and cope with the consumer's challenges.
  • Provide assurance to families that the team is working towards the consumer's goals.
  • Identify a clear picture of the patient's pre-injury lifestyle, abilities, skill sets, and interests to enable the team to approach the patient in a meaningful way.
  • Ensure a smooth transition for individuals on caseload as they discharge from our program to home or another level of care.
  • Provide and/or coordinate psycho-emotional services to individuals as a primary therapist.
  • Facilitate clinical reviews and team meetings as applicable.
  • Function as a case manager to coordinate internal and external services provided to individuals and families.

Requirements:

  • Master's degree in Social Work (M.S.W.) from an accredited college or university.
  • Licensed as a Social Worker by the State of Michigan.
  • Experience in a health/human service field with an emphasis in rehabilitation and discharge planning.
