Key Responsibilities:

  • Assess patients with complex wounds, ostomies, and/or continence conditions and develop individualized care plans.
  • Collaborate with nursing, rehabilitation, and medical staff to ensure effective wound care treatment.
  • Provide education and training to staff on wound care best practices.

Requirements:

  • Must be a licensed LPN in good standing with the state (as required).
  • 1-2 years wound care experience required.
  • Wound Care Certification (WOCN) preferred.
  • 1+ years of long-term care experience preferred.
  • Reliable transportation required.
