About the Role:
We are currently seeking a dedicated and compassionate Social Worker to join our team at Transitions Care. In this role, you will work closely with patients, families, and healthcare professionals to provide exceptional psychosocial support and care. Responsibilities:
• Develop and implement comprehensive care plans that address patients' and families' unique needs
• Coordinate with interdisciplinary teams to ensure seamless care delivery
• Identify and access community resources to support patients and families
• Maintain accurate and timely documentation of patient interactions and progress
• Participate in continuing education and professional development opportunities

Requirements:
Bachelor's degree in Social Work, Master's degree preferred. Minimum 2+ years social service experience in a healthcare setting or social services agency. Licensure and/or certification as required by the state. Strong knowledge of local community resources.
