Case Management RN Specialist

We are seeking a highly skilled Case Management RN Specialist to join our team at Kaiser Permanente in Orange, California. This is an excellent opportunity to work with a talented group of professionals who share your passion for delivering exceptional patient care.
Job SummaryThis RN Case Manager will play a critical role in managing the specialized needs of patients, working collaboratively with physicians and other healthcare providers to ensure seamless care coordination. The successful candidate will have a strong clinical background, excellent communication skills, and the ability to navigate complex healthcare systems.
Key Responsibilities- Assess patient needs and develop comprehensive care plans that address specific requirements.
- Collaborate with physicians and other healthcare providers to coordinate care and ensure effective treatment.
- Monitor patient outcomes and adjust care plans as needed to achieve optimal results.
- Communicate effectively with patients, families, and caregivers to educate them on their conditions and promote self-management.
- Develop and implement protocols to improve patient care and outcomes.
To be successful in this role, you must have:
- A minimum of two years of experience as an RN in an acute care or ambulatory care setting.
- A bachelor's degree or equivalent experience.
- Current RN licensure in the state of California.
- Demonstrated ability to utilize and apply general and specialized principles, practices, techniques, and methods of utilization review/management, care coordination, transfer coordination, discharge planning, or case management.
- Working knowledge of regulatory requirements and accreditation standards (TJC, Medicare, Medi-Cal, etc.).
- Computer literacy skills and proficiency in electronic medical records.
- Bilingual (English/Spanish) Level II required.
Certificate in case management or specialty certification preferred.
Salary Range$115,000 - $140,000 per year, depending on experience.
