Aveanna Healthcare is seeking a skilled and compassionate Home Health Nurse - RN to join our team. As a Private Duty Nurse, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes.
Responsibilities- Implement and manage patient care plans in accordance with physician orders and Aveanna's policies and procedures.
- Conduct thorough patient assessments and coordinate care with other healthcare professionals as needed.
- Administer prescribed medications, treatments, and therapies as ordered by physicians.
- Document patient care and progress in a timely and accurate manner.
- Provide education and training to patients and their families on health promotion, disease prevention, and management of chronic conditions.
- Collaborate with the Aveanna Care Team to ensure seamless care transitions and optimal patient outcomes.
- Active, unencumbered RN license in the state of practice.
- Current CPR certification from the American Red Cross or American Heart Association.
- TB skin test (current within the last 12 months).
- At least six months of prior hands-on nursing experience (preferred but not required).
- Reliable transportation.
