Position Highlights:
*Advise the Nursing Home Administrator, Director of Nursing, and Food Service Director on all clinical aspects of nutrition care, adhering to standards set by regulatory agencies and licensing boards.
*Collaborate with the interdisciplinary team to formulate nutrition care plans for residents/patients.
*Monitor and assess the effectiveness of nutritional interventions.
*Ensure timely and accurate documentation of nutrition assessment tools, recommended interventions, and follow-up actions.
*Address nutrition-related concerns with all members of the interdisciplinary team.
*Provide dietary guidance and education for residents/patients, families, and/or caregivers.
*Oversee all aspects of the nutrition care delivery process to ensure clinical excellence.
Qualifications:
*Registered and/or licensed Dietitian, as mandated by the state of employment. *Previous experience in a healthcare environment or as a consulting dietitian is preferred. *Experience with the geriatric population is advantageous.
