Responsibilities- Provide admission, case management, and follow-up skilled nursing visits to patients in their homes.
- Develop and implement individualized care plans in consultation with physicians and other healthcare professionals.
- Confer with physicians to develop and revise care plans based on ongoing assessments and patient progress.
- Provide hands-on care, education, and support to patients and their families, promoting independence and optimal health outcomes.
- Collaborate with other healthcare professionals, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive care.
- Document patient care and progress accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum of one year of nursing experience, preferably in a home health or acute care setting.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license and reliable transportation.
