As a LPN at Sunrise, you will play a vital role in delivering high-quality care to our residents, ensuring their physical, emotional, and social needs are met. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor resident health, and provide education and support to residents and their families.
Responsibilities:- Conduct resident assessments and develop care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as prescribed by physicians
- Monitor resident health and report any changes or concerns to the Resident Care Director
- Provide education and support to residents and their families on health and wellness topics
- Collaborate with the interdisciplinary team to develop and implement quality improvement initiatives
- Graduate of an accredited LPN program
- Current state license as a LPN/LVN
- Minimum of one year experience in a healthcare setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
