Job Description

We are seeking an experienced and dedicated Home Health Clinical Manager to lead our clinical team in Washington, DC. The ideal candidate will have a strong background in home healthcare management, clinical operations, and staff leadership, with a passion for driving quality outcomes and patient satisfaction.

Responsibilities
  • Clinical Leadership: Provide strategic direction and oversight to the clinical team, ensuring the delivery of high-quality, evidence-based care to patients in accordance with regulatory standards and best practices.
  • Staff Management: Recruit, onboard, and supervise clinical staff, including registered nurses, licensed practical nurses, and home health aides, fostering a culture of excellence, collaboration, and continuous learning.
  • Performance Optimization: Develop and implement performance improvement initiatives to enhance clinical efficiency, productivity, and outcomes, while minimizing risks and ensuring patient safety.
  • Quality Assurance: Establish and maintain quality assurance programs to monitor and evaluate clinical performance, outcomes, and compliance with regulatory requirements, accreditation standards, and organizational policies.
  • Care Coordination: Collaborate with interdisciplinary teams, including physicians, therapists, social workers, and case managers, to coordinate care plans, facilitate transitions of care, and optimize patient outcomes across the continuum.
  • Patient Advocacy: Serve as an advocate for patients and their families, addressing their needs, concerns, and preferences, and promoting a patient-centered approach to care delivery.
  • Documentation and Reporting: Oversee the timely and accurate documentation of patient assessments, care plans, progress notes, and clinical outcomes, ensuring compliance with documentation standards and confidentiality requirements.
  • Budget Management: Assist in the development and management of the clinical department budget, monitoring expenses, optimizing resource allocation, and identifying opportunities for cost containment and revenue enhancement.
Qualifications
  • Bachelor's degree in Nursing or related field required; Master's degree preferred.
  • Current Washington, DC state licensure as a Registered Nurse (RN).
  • Minimum of 7 years of clinical experience in home healthcare or a related setting, with at least 2 years in a clinical management role.
  • Demonstrated expertise in clinical operations, staff management, and quality improvement initiatives.
  • Strong knowledge of Medicare/Medicaid regulations, home health accreditation standards, and industry best practices.
  • Excellent communication, interpersonal, and leadership skills, with the ability to motivate and inspire a diverse team.
  • Proficiency in electronic health records (EHR) systems and other relevant software applications.
  • Commitment to upholding ethical standards, integrity, and professionalism in all aspects of work.
