Role Overview for Physical Therapist:

The Physical Therapist plays a vital role within the rehabilitation department of a skilled nursing facility, concentrating on improving residents' physical health and functional recovery.

Key Responsibilities:
  • Assessment and Evaluation: Conduct comprehensive physical assessments to identify residents' impairments and limitations, with a focus on mobility, balance, and gait.
  • Individualized Care Plans: Develop tailored treatment plans based on assessments, establishing specific goals to enhance residents' mobility and reduce discomfort.
  • Data Collection: Collect essential data accurately for various assessments, ensuring precision for different evaluation types.
  • Treatment Implementation: Utilize a variety of physical therapy techniques, including exercises and manual interventions, to improve residents' physical capabilities.
  • Patient Education: Educate residents, family members, and caregivers on safe mobility practices and exercises for ongoing improvement. Employ remote therapeutic monitoring for follow-up after discharge.
  • Documentation: Maintain proficiency in Point of Service (POS) care procedures and documentation. Keep detailed records of evaluations, treatment plans, and progress notes in electronic medical records (EMRs).
  • Collaborative Care: Work in coordination with the rehabilitation team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive resident care.
