Minimum Qualifications
- You must hold a Doctor of Optometry degree from an accredited school.
- You must possess a valid state license in Optometry.
- For Ophthalmologists (MD or DO), you must hold a valid state license.
- You must provide a General Liability Certificate.
- You must show proof of Professional Liability Insurance.
- You must submit a W-9 form.
