Role Overview

This role is responsible for coordinating the care and services of select patients. The ideal candidate will have excellent communication skills, be able to work independently, and possess a strong understanding of healthcare systems and policies.

The successful candidate will also have experience working with diverse patient populations, including those with complex psychosocial needs. They will be able to assess patients' needs, develop effective plans, and coordinate care with other healthcare providers.

This role requires strong organizational skills, attention to detail, and the ability to prioritize tasks effectively. The successful candidate will be able to work in a fast-paced environment and maintain confidentiality when dealing with sensitive patient information.

Key Responsibilities
  • Coordinate the care and services of select patients
  • Assess patients' needs and develop effective plans
  • Coordinate care with other healthcare providers
  • Provide education and support to patients and families
  • Maintain accurate and up-to-date records
Requirements
  • Master's degree in social work (MSW) or related field
  • Current state licensure as a Social Worker (LSW, LISW or LISW-S)
  • Basic Life Support (BLS) certification through the American Heart Association (AHA)
  • Flexible, open-minded and adaptable to change
Preferred Qualifications
  • Certification as a Case Manager within one year of eligibility from an approved professional organization
  • Three years of experience in a healthcare system that includes exposure to and experience in pre- and post-acute care
  • Hospital social work experience
  • Homecare and/or hospice social work experience
  • Knowledge of community services
