Job Summary

We are seeking a compassionate and dedicated Aftercare Case Manager to join our team at TFI Family Services, Inc. As an Aftercare Case Manager, you will provide support and services to children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth.

Key Responsibilities:

  • Provide crisis intervention for children and families as needed
  • Work closely with public and private agencies providing supportive services
  • Maintain accurate and timely documentation and reports
  • Coordinate community and educational services
  • Meet with children and families as determined by the aftercare plan to support reunification and address concerns

Requirements:

  • Bachelor's degree in human services
  • Valid driver's license
  • Ability to work with children and families in a supportive and non-judgmental manner
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team

What We Offer:

  • Competitive hourly rate of $22.00
  • $1,000 sign-on bonus
  • Hybrid work capabilities for those who reside in or around Chanute area
  • 20 days of personal leave in the first year of employment and 25 days after completion of year one
