Job Title: Social Worker - Behavioral Health

Sharon Hospital is seeking a Full-Time Social Worker to join its Behavioral Health Team. As a key member of our team, you will provide social service and discharge planning functions for the inpatient behavioral health unit.

Responsibilities:
  • Conduct psychosocial assessments within 72 hours of admission.
  • Identify and address psychosocial needs of patients and families, providing support, counseling, and crisis intervention.
  • Collaborate with physicians to communicate social service, post-hospital care, and discharge planning needs.
  • Lead insight-oriented, psychoeducational, process, or skills groups.
  • Run family meetings with physicians and patients as needed.
  • Provide support with advance directives and assist patients and families with the grieving process, making referrals to Pastoral Care as appropriate.
  • Determine and communicate the type of assistance, setting, or resources necessary for patients and families, providing appropriate resources.
  • Resolve personnel concerns at the departmental level, utilizing the appropriate processes as required.
  • Maintain safety and regulatory requirements, including state, federal, and Joint Commission regulations.
  • Document all interactions and activities in accordance with current standards and policies.
  • Maintain current knowledge of resources available within the community and supply resource materials to patients as needed.
  • Act on performance improvement issues identified by the behavioral health leadership team.
  • Maintain and model REACH Values (Respect, Excellence, Accountability, Compassion, Honor).
  • Demonstrate regular, reliable, and predictable attendance.
Requirements:
  • Master's degree in Social Work (MSW).
  • Licensed Master Social Worker (LMSW).
  • Minimum of three years' experience in all aspects of social service and discharge planning.
  • Prefers two years of social work experience.
Preferred Qualifications:
  • Bilingual.
  • Knowledge of Microsoft Office, including Word, Excel, PowerPoint, and Outlook.
  • Basic computer knowledge.
  • Excellent customer service skills.
  • Ability to multitask, make decisions, and communicate effectively.
License, Registration, or Certification Requirements:
  • Connecticut Licensed Clinical Social Worker (LCSW) or license eligible.
