As a key member of our team, you will be responsible for delivering high-quality patient care and collaborating with our healthcare professionals to ensure the best possible outcomes.

Responsibilities:

  • Conduct daily assessments of clients to identify their medical needs and develop personalized care plans.
  • Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
  • Document client status and changes in their condition, maintaining accurate and detailed records.
  • Communicate effectively with our healthcare team, including RNs, MDs, and other medical professionals, to ensure seamless care coordination.
  • Provide tracheostomy care and suctioning, as well as tube feeding and disease management, as needed.
  • Coordinate client medical appointments and ensure timely follow-up care.

Requirements:

  • At least two years of experience as an LPN, with a strong background in ICU/PCU or tracheostomy care.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong attention to detail and ability to maintain accurate records.
