Position Overview

We are seeking a qualified Palliative Care Nurse Practitioner to become a vital part of our team. In this role, you will report directly to the Executive Director or Administrator.

Your responsibilities will include conducting Face-to-Face (F2F) assessments and gathering essential clinical data to support hospice eligibility determinations.

The information collected will be communicated to the hospice physician responsible for certifying or re-certifying the patient's eligibility for hospice services.


The role of the hospice Nurse Practitioner is specifically focused on tasks necessary to complete F2F encounter visits.


  • Conducts and finalizes physical evaluations for the F2F encounter within the regulatory timelines set by CMS.
  • Provides clinical insights, including a review of medical treatment plans, diagnostic evaluations, laboratory results, and medication lists to the physician overseeing the patient's hospice eligibility.
  • Offers appropriate recommendations to the Interdisciplinary Group (IDG) based on the findings from the F2F assessment.
  • Informs the hospice agency if unable to complete a F2F visit as scheduled.
Qualifications

  • Master’s degree or higher in Nursing.
  • Completion of an accredited graduate-level nurse practitioner program.
  • Valid state license as a registered nurse.
  • Current state license or certification as an advanced practice nurse.
  • National certification as a Nurse Practitioner (ANCC or AANP).
  • A collaborative agreement must be in place.
