Clinical Pharmacist Job Description

We are seeking a highly skilled Clinical Pharmacist to join our team at Phoenix Children's Hospital. In this role, you will be responsible for providing comprehensive pharmacy services to patients from neonate to young adulthood.

Key Responsibilities:

  • Manage medication orders for therapeutic appropriateness and intervene as necessary.
  • Verify medication orders accurately and timely.
  • Screen for and manage clinical alerts/conditions such as allergies, interactions, therapeutic duplications, and adverse drug reactions.
  • Problem-solve issues with medication orders and contact prescribers to recommend alternative medication therapy when necessary.
  • Comply with the directives of the Pharmacy and Therapeutics Committee related to Formulary Management.
  • Document clinical interventions to meet or exceed established cost containment and evidence-based medication therapy goals.
  • Prepare and dispense sterile and non-sterile medications accurately and timely.

What We Offer

We offer a competitive salary range of $120,000 - $150,000 per year, depending on experience, along with opportunities for professional growth and development.
