Job Summary
Catholic Health is seeking a dedicated and compassionate Care Manager-PD to join our team. As a Care Manager-PD, you will play a vital role in assessing, planning, implementing, and evaluating options and services to effect an appropriate individualized plan of care for patients across the acute care continuum.
Key Responsibilities
- Assess patients' needs and develop individualized care plans
- Coordinate with healthcare teams to ensure seamless care transitions
- Monitor patients' progress and make adjustments to care plans as needed
- Collaborate with patients, families, and caregivers to ensure informed decision-making
Requirements
- NYS RN License
- Recent med/surg or cardiac experience
- PRI and/or case management certification preferred
