Behavioral Health Coach

As a Mental Health Professional-Licensed, you'll be an integral part of our team, working closely with patients to develop personalized behavioral change plans and facilitate group and individual therapy sessions.
Estimated Salary: $64,000 - $82,000 annually, depending on experience and qualifications.
Job Summary: We're seeking a highly skilled and compassionate Mental Health Professional to join our team. As a key member of our primary care team, you'll work collaboratively to assess patients' clinical status, develop treatment plans, and monitor patient care.
Essential Duties and Competencies:
- Skills Training and Development: Provide skills training through psycho-education and patient education strategies, and develop specific behavioral change plans for clients.
- Therapy Sessions: Facilitate group therapy sessions as well as individual sessions with patients.
- Collaboration with Primary Care Providers: Assist primary health care providers in recognizing and treating mental disorders and psycho-social problems.
- Patient Assessment: Assess the clinical status of patients referred by primary care providers through brief consultative contacts.
- Treatment and Management: Work with the primary care team to treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
- At-Risk Patient Detection: Assist in the detection of at-risk patients and the development of plans to prevent further psychological or physical deterioration.
- Evaluation and Improvement: Evaluate patient care plans with the primary care team, monitor the site's behavioral health program, and make recommendations for improvement.
- Education and Support: Teach patients, families, and staff about care, prevention, and treatment enhancement techniques.
Minimum Training and Experience Requirements:
- Clinical Licensure: Current state licensure as a Psychiatric Nurse, Licensed Independent Clinical Social Worker (LICSW), Licensed Professional Clinical Counselor (LPCC), Licensed Psychologist (LP), and Licensed Marriage and Family Therapist (LMFT).
- Experience: A minimum of two years of freedom from chemical abuse problems.
