Position Overview:
The Clinical Surgery Coordinator plays a pivotal role in collaborating with the healthcare team and medical professionals to facilitate effective discharge planning and intervention for surgical patients. This position is dedicated to enhancing patient outcomes and optimizing the use of healthcare resources for individuals with intricate health challenges.
Key Responsibilities:
Assessment:
Conducts comprehensive and ongoing assessments to identify care coordination and discharge planning requirements based on the patient's clinical status and needs.
Planning:
Develops a targeted discharge plan for high-risk patients, ensuring all identified needs are addressed effectively.
Intervention and Evaluation:
Works closely with the clinical team and medical providers to implement the discharge plan, ensuring a smooth transition for patients.
Performance Improvement:
Stays informed about evolving organizational practices, regulatory changes, and third-party payer requirements that impact clinical care and discharge processes.
Qualifications:
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) required.
- 3-5 years of relevant experience, with a minimum of 2 years in Care Management focused on Surgery or Intensive Care Units (ICU) preferred.
- Active Maryland Registered Nurse License.
- Intent to obtain Maryland licensure if currently licensed in another state.
- Case Management certification preferred.
