Requirements:
Must possess an active Washington state nursing license to be considered. Please provide your WA license status: Active, In Process, or No License on your profile. If in process, include the application date and estimated issue date.
Travelers who have previously worked as permanent staff at any Providence-affiliated facility must have a separation period of at least 3 months to qualify for a travel contract.
We welcome applications from MSW Care Managers (with either an Associate clinical license or LCISW) or RN Care Managers. Candidates must hold a Master's in Social Work and a Washington-issued post-MSW license, such as the licensed social work associate independent clinical. It is essential to confirm that the clinician has the required documentation upon submission.
Experience:
Candidates should have case management and discharge experience. Utilization management experience is beneficial but not mandatory.
Average Daily Census: 18-20
Essential Skills- General Skills: Experience in Acute Hospital settings, working with patients with communication challenges, and familiarity with community programs.
- Case Management: Proficiency in admission criteria, initial evaluations, care coordination, and discharge planning.
- Documentation: Ability to document care delivery according to the plan of care and coverage criteria.
- Major Disease Processes: Experience with conditions such as Dementia and Stroke is required.
