PACS Inc is seeking a skilled MDS Coordinator (RN) to join our team at Sunrise Post Acute. As a key member of our healthcare team, you will be responsible for interviewing residents, family, and healthcare professionals to gather information for the Minimum Data Set (MDS).Key Responsibilities:
• Conduct thorough interviews with residents, family, and healthcare team members to gather information for the MDS.
• Review medical records to ensure accuracy and completeness of information.
• Observe and assess residents to identify their needs and preferences.
• Complete and transmit the MDS according to regulatory time frames.
• Schedule MDS assessments and ensure timely completion.
• Develop and implement care plans that meet the unique needs of each resident.
• Collaborate with the Healthcare Common Procedure Coding System (HCPCS) team to facilitate the completion of the MDS.
• Participate in care plan meetings with residents, family, and healthcare professionals to ensure that care plans are up-to-date and effective.
• Demonstrate knowledge of Medicare reimbursement, PDPM rates, and PPS systems to ensure accurate and compliant care planning.
• Communicate the Validation report to the Administrator and Director of Nursing (DON).
• Possess knowledge of ICD codes and other relevant coding systems.
Requirements:
• Previous experience as an MDS Coordinator (RN) in a skilled nursing facility (SNF) or long-term care (LTC) setting.
• At least 1 year of experience in a SNF/LTC setting.
• Current California RN Nursing License in good standing.
