About the Job

We are seeking a highly motivated and experienced LPN to join our team. As a Licensed Practical Nurse (LPN) with IntelyCare, you will have the opportunity to work with a variety of patients and families, providing high-quality care and support in a fast-paced and rewarding environment.

Responsibilities
  • Deliver post-acute care to patients in a variety of settings
  • Monitor patients' physical and mental well-being, tracking vital signs and documenting health concerns
  • Perform intakes of patients, documenting their health concerns and medical histories
  • Review medical records and charts to ensure accurate and up-to-date information
  • Administer medications and verify their accuracy
  • Help maintain a safe and comfortable environment for patients
  • Assist medical staff with orientation and training
Qualifications and Skills
  • Current New Hampshire state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school diploma or equivalent
  • Ability to lift a minimum of 50 pounds
  • Minimum of 6 months of experience as an LPN or in a related role
