Job Title: Physical Therapist

Pinnacle Home Care Inc. is seeking a skilled Physical Therapist to join our team of dedicated healthcare professionals. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Job Summary:

The Physical Therapist will be responsible for assessing patients' conditions, developing personalized treatment plans, and delivering therapy to improve mobility, reduce pain, and restore function. You will work closely with our interdisciplinary team to ensure seamless care and excellent patient outcomes.

Key Responsibilities:
  1. Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  2. Develop and implement treatment plans in consultation with physicians and revise as necessary.
  3. Assist in developing and implementing the interdisciplinary client care plan as it pertains to Physical Therapy.
  4. Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light, and electricity.
  5. Teach, supervise, and counsel the family and client in the physical therapy program.
  6. Record all evaluation data, treatments, and client's response to therapy intervention on a timely basis.
Requirements:
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • Home care experience and at least one year of supervisory experience (2 years preferred).
  • Ability to drive and access clients' homes, which may not be routinely wheelchair accessible.
  • Hearing, eyesight, and physical dexterity sufficient to perform a physical assessment of the client's condition and to perform client care.
