About the RoleThis is a unique opportunity to join our team of compassionate professionals who are passionate about making a difference in the lives of our patients. As a Clinical Assessment Advisor, you'll be responsible for:
- Conducting thorough assessments of patient needs, identifying areas for improvement, and developing personalized care plans
- Collaborating with our interdisciplinary team to ensure timely and effective care delivery
- Providing education and support to patients and their families, empowering them to take an active role in their care
- Monitoring patient progress, making adjustments to care plans as needed, and ensuring seamless transitions between care settings
- Participating in quality improvement initiatives, contributing to the development of best practices, and promoting a culture of excellence within our organization
To be successful in this role, you'll need:
- A Bachelor of Science degree in Nursing (BSN) or a related field
- At least 2 years of clinical experience in home health care, with a minimum of 1 year in a leadership or management role
- Current CPR certification and a valid driver's license
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
- Knowledge of state and federal regulations governing home health care, as well as industry best practices
