Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Hudson Headwaters Health Network. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Key Responsibilities
  • Provide Patient Care: Administer care to patients under the direction of the responsible Registered Nurse or medical provider, ensuring a safe and compassionate environment.
  • Collaborate with Healthcare Team: Work collaboratively with staff and other departments to develop strategies for improving quality of care and the patient experience.
  • Utilize Data for Care Coordination: Use data to continuously meet care coordination needs across settings, in conjunction with the patient, family, and/or other care providers.
  • Protect Patients and Employees: Adhere to infection control policies and protocols to protect patients and employees.
  • Prepare Patients for Encounters: Follow established protocols to prepare patients for encounters.
  • Demonstrate Clinical Competence: Demonstrate competence in clinical skills and proper medication and vaccine management processes.
  • Document Patient Information: Document patient information accurately and timely in the electronic medical record.
  • Respond to Patient Calls: Respond to patient phone calls and engage the provider or RN as needed.
  • Reinforce Patient Education: Reinforce patient and family education plans that meet the needs of the patient.
  • Perform Point of Care Testing: Perform ordered point of care testing, including specimen collection, and proper documentation.
  • Assist with Patient Encounter Flow: Assist with patient encounter flow and trouble-shooting issues in collaboration with the supervisor.
  • Provide Guidance and Mentorship: Provide guidance and mentorship to new and incumbent staff based on skill set.
  • Attend Meetings and Committees: Attend meetings and serve on various organization committees.
  • Assist with Compliance and Staff Development: Assist the supervisor with compliance and staff development duties.
  • Manage Medical Supplies and Equipment: Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
  • Perform Additional Duties: Perform additional duties as assigned.
Requirements
  • Unrestricted LPN License: Unrestricted current LPN license to practice nursing in New York State.
  • Certificate from an Accredited School of Nursing: Must possess a certificate from an accredited school of nursing.
  • Minimum of 1-Year Clinical Experience: Minimum of 1-year clinical experience preferred.
  • Ability to Travel: Ability to travel throughout the Network.
  • Current BLS Certification: Current BLS certification.
  • Proficiency with Microsoft Applications and Electronic Medical Records: Proficiency with Microsoft applications and electronic medical records.
  • Excellent Verbal and Written Communication Skills: Excellent verbal and written communication skills.
  • Self-Directed, Detail-Oriented, Motivated, and Ability to Work Independently and in a Team Atmosphere: Self-directed, detail-oriented, motivated, and ability to work independently as well as in a team atmosphere.

Pay Rate: $28/hour
