Job Summary

The MSW Case Manager plays a critical role in the complex discharge planning process, collaborating with physicians and healthcare teams to assess, plan, implement, and evaluate proactive, complex, and safe discharge plans.

With a strong background in social work, this individual will facilitate coordination of care, serving as a resource for discharge planning and care coordination for psychosocially complex cases.

Maintaining current knowledge of available community resources and post-acute care options, the MSW Case Manager will ensure seamless transitions for patients, prioritizing their needs and well-being.

Key Responsibilities
  • Collaborate with healthcare teams to develop and implement discharge plans
  • Facilitate coordination of care, ensuring seamless transitions for patients
  • Maintain current knowledge of community resources and post-acute care options
  • Provide cultural information to facilitate support for treatment plans
  • Ensure accurate communication of patient information, including outpatient services, hospitalization, and treatment plans
Requirements
  • Master's Degree in Social Work from a Council on Social Work Education-accredited program
  • 2+ years of professional social work experience or 2+ years of healthcare experience, with case management or related experience preferred
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Ability to lift/push/pull up to 20 lbs occasionally, with frequent sitting, standing, walking, and bending/stooping
