Home Risk Assessment Specialist

Matrix Medical Network is seeking a skilled Home Risk Assessment Specialist to join our team.

We offer a competitive salary of $90.00 per completed assessment, with opportunities for flexible scheduling and mileage reimbursement.

The Home Risk Assessment Specialist will conduct comprehensive health assessments in patients' homes, reviewing their medical history, medication adherence, social environment, and other risks. This provides unparalleled insight into a patient's overall health and well-being, enabling us to identify potential care gaps and improve quality of care.

• Conduct home-based health assessments, including medical history, diagnosis, and treatment recommendations.
• Collaborate with physicians, case managers, and other healthcare professionals to develop personalized care plans.
• Provide education and support to patients and their families to promote healthy behaviors and disease management.
• Document all assessments and interventions in accordance with Matrix Medical Network guidelines and protocols.

• Masters degree in Nursing required; OR commensurate experience and satisfactory completion of NP licensure.
• Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
• Minimum 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with electronic health records.
• Excellent verbal and written communication skills.
