Job Summary

We are seeking a highly motivated and experienced Field Housing Navigator to join our team in the Southeast Region of Michigan. As a Field Housing Navigator, you will play a critical role in connecting low-income individuals and families with safe and affordable housing options.

Key Responsibilities
  • Conduct screening interviews and complete intake documentation to identify housing needs and develop personalized plans for individuals and families.
  • Collaborate with national UnitedHealthcare teams to develop a community-based housing coordination strategy that addresses the unique needs of our members and the community.
  • Develop and implement processes to manage the intake and referral of individuals experiencing housing issues, including coordinating with local housing authorities, non-profit organizations, and other stakeholders.
  • Engage with individual member cases to identify appropriate housing options, including subsidized housing, Section 8, and affordable and market-rate housing, and provide support services to help individuals maintain stable housing.
  • Build relationships with housing-focused community organizations, landlords, and affordable housing developers to identify new and existing opportunities for accessing resources and support services.
  • Coordinate with other social determinants of health coordinators to facilitate access to support services, such as employment, transportation, and financial services, to help individuals achieve stable housing and overall well-being.
Requirements
  • Bachelor's degree in Business, Health Administration, Public Policy, Social Work, or a related field, or Associate degree with 2+ years of experience in housing agencies, housing policy, local housing organizations, public housing authorities, housing subsidies/vouchers, and other low-income housing support programs.
  • 2+ years of experience with housing agencies, housing policy, local housing organizations, public housing authorities, housing subsidies/vouchers, and other low-income housing support programs, specifically in Michigan.
  • Intermediate level of experience with project/program management, including action planning, timelines, and business requirement definition.
  • Intermediate level of proficiency in PowerPoint.
  • Demonstrated resiliency and tenacity in driving solutions for low-income individuals seeking supportive services, including conflict management, influencing without authority, and problem-solving beyond prescriptive solutions.
  • Demonstrated ability to network, build connections, and develop strong relationships in the community.
  • Strong understanding of low-income and/or Medicaid populations with complex needs, including individuals experiencing chronic homelessness and behavioral health conditions.
  • Strong leadership skills with the ability to work independently and with minimal direction.
  • Strong communication and presentation skills.
  • Sensitivity to individual needs and a strong person-centered orientation.
  • Ability to work in a complex matrix environment, influencing others through communication and organizational skills.
Preferred Qualifications
  • Housing or Affordable housing experience.
  • Understanding of Medicaid and Medicare programs, as well as managed care.
  • Bilingual.
  • Healthcare experience.
