Job Summary

We are seeking a skilled Nurse Practitioner (NP) to join our team at Abode Care Partners. As a Medical Care Specialist - Advanced Practice, you will be responsible for performing evidence-based medical care, managing chronic and urgent episodic illnesses, and collaborating with other healthcare professionals to provide interdisciplinary care.

  • Perform routine medical examinations and treatments, including ordering laboratory and diagnostic testing as needed
  • Develop and implement comprehensive plans of care to meet the unique needs of each patient
  • Collaborate with caregivers and other healthcare professionals to ensure seamless coordination of care
  • Provide education and support to patients and their families to promote healthy lifestyles and disease management
Requirements

To be successful in this role, you must possess:

  • A Master's degree in Nursing (MSN)
  • An active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • At least two years of nurse practitioner experience preferred
