Responsibilities
As a Home Health Nurse with minimal travel, our Registered Nurses (RNs) oversee, assess, and deliver nursing services to clients in various senior living settings, including assisted living and independent living facilities.
Our facility-based RNs appreciate:
- Providing care to patients within the welcoming environment of assisted living facilities
- 1:1 patient care
- The opportunity to follow each patient from admission to discharge
Enhabit RNs collaborate daily with facility wellness teams to ensure effective communication and continuity of care for residents and their families.
Additionally, our facility-based RNs consult with physicians and office staff as necessary, providing updates on patient care and serving as the primary contact for all disciplines involved in patient care.
Key responsibilities of our RN Case Managers include performing accurate OASIS assessments, maintaining current medication profiles, ensuring timely reporting of lab values to physicians, and participating in weekly case conferences and monthly case manager meetings.
Qualifications
Registered Nurses (RN)
Case Managers must meet the following criteria:
- Hold a current Registered Nurse (RN) license in the state of employment
- Possess a minimum of one year of clinical experience (preferred)
- Demonstrate knowledge and proficiency in current nursing practices
- Have a valid state driver's license and automobile liability insurance
