About the Role

We are seeking a skilled Medical Social Worker to join our team at Assured Hospice, a part of LHC Group. As a Medical Social Worker, you will play a vital role in providing medical social services to patients and their families, working under the direction of a physician and Interdisciplinary Group.

Key Responsibilities
  • Assist patients, significant others, physicians, and healthcare team staff in understanding significant personal, emotional, environmental, and social factors related to the patient's health status.
  • Contribute to the development of a comprehensive, integrated Plan of Care for patients daily.
  • Instruct healthcare team members on community resources available to assist patients as needed.
  • Function as a Bereavement Coordinator and supervise the provision of bereavement services reflective of patient family needs, establishing a Plan of care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery up to thirteen (13) months following the death of the patient.
Requirements
  • Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
  • One year of social work experience in a healthcare setting.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • WA: If not licensed as a social worker, Agency Affiliated Counselor credential is required.
