We are seeking a highly skilled Registered Nurse (RN) to join our team as a Home Health Care Professional. This role involves making home visits to clients in designated geographic territories, administering medication, wound care, treatments, and procedures as necessary.

  • This is a staff nursing position that requires strong interpersonal skills, solid computer skills, and the ability to work independently and manage time effectively.

The ideal candidate will have a current license as a Registered Nurse in Vermont, a minimum of one year of recent clinical experience, and prior home care experience is strongly preferred.

Job Responsibilities
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures as necessary.
  • Make home visits to clients in designated geographic territories.
  • Monitor clients' conditions and report changes to Clinical or Client Services Manager.
  • Follow up with doctors' orders and accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
  • Case management and coordination.
