About the Role:

Job Summary

The Hospice Care Specialist is accountable for delivering exceptional patient care in alignment with established standards across diverse environments, including residential settings, healthcare facilities, and long-term care institutions. This role necessitates collaboration with various healthcare professionals and members of the hospice interdisciplinary team to ensure effective patient care delivery and the realization of optimal patient outcomes. The specialist will incorporate knowledge of the patient's age, gender, and cultural background into the care provided. The role also includes supervising hospice aides and providing on-call support for the hospice program.

Minimum Job Qualifications

  • Licensure or other certifications: Valid RN licensure in Georgia. Current BLS certification.
  • Educational Requirements: Associate's Degree in Nursing.
  • Minimum Experience: At least one (1) year of clinical nursing experience.
  • Other:

Preferred Job Qualifications

  • Preferred Licensure or other certifications: CHPN.
  • Preferred Educational Requirements:
  • Preferred Experience: Experience focusing on care for terminally ill patients, adults in acute care settings, and home care. Additional experience with pediatric patients is advantageous. One (1) year of hospice experience is preferred.

Essential Knowledge, Skills, and Abilities

  • Strong clinical assessment and nursing skills
  • Ability to interpret and adhere to regulations, agency policies, and accreditation standards
  • Excellent interpersonal and customer service skills
  • Proficient in documenting clinical care using electronic systems
  • Able to work autonomously and collaboratively within a team
  • Frequent travel within the service area; must possess a valid driver's license and current auto insurance with a minimum of $100,000 liability. Availability for 24-hour on-call assignments as scheduled.

Key Responsibilities

  • Conducts patient admissions to the hospice program, ensuring compliance with state and federal regulations and hospice admission criteria. Completes comprehensive assessments during the admission process.
  • Delivers nursing care in accordance with the standards set forth by the American Nurses Association (ANA) and the Standards of Hospice Nursing Practice. Performs various types of visits as needed, implementing and evaluating care plans.
  • Maintains accurate documentation of patient care in line with medical standards and agency policies, ensuring timely and concise charting.
  • Facilitates communication among the patient, family, primary nurse, and interdisciplinary team to ensure coordinated care and effective case management.
On-Call Responsibilities

  • Provides after-hours on-call coverage, responding to patient and family needs with professionalism and compassion. Visits patients as necessary based on call requirements.
  • Participates in quality improvement initiatives and peer reviews, documenting any variances or incidents in accordance with agency policies.
  • Engages in ongoing professional development, attending required meetings and educational programs to maintain clinical expertise.
