Hospice Care Nurse Job Summary

Affinity Care of Ohio is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our hospice care team. As a Hospice Care Nurse, you will provide high-quality patient care and support to individuals and families in their homes and communities.

Key Responsibilities:
  • Conduct routine nursing assessments to determine patient needs and develop individualized care plans.
  • Provide nursing care and interventions to meet patient needs, including administering medications and treatments as prescribed.
  • Collaborate with the interdisciplinary team to coordinate patient care and ensure continuity of services.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Participate in on-call duties and ensure timely responses to patient needs.
Requirements:
  • Graduate of an accredited school of nursing.
  • LPN license in good standing to practice in the state of Ohio.
  • Minimum of 2 years of experience as a nurse within the last 3 years, or 1 year of experience in public health, home care, or hospice nursing.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, and basic math skills.
  • Ability to lift, position, and transfer patients, as well as lift supplies and equipment.
