About the Role

We are seeking a highly skilled and compassionate Case Manager to join our team at Volunteers of America of Massachusetts. As a Case Manager, you will play a critical role in providing day-to-day support to residents in our Recovery Services Residential program, ensuring a safe and secure therapeutic environment.

Key Responsibilities
  • Facilitate a Safe and Supportive Environment: Create a welcoming and inclusive atmosphere for residents, promoting a sense of community and belonging.
  • Weekly Case Management Sessions: Conduct regular one-on-one sessions with up to 9 residents, providing guidance and support to help them achieve their goals and overcome challenges.
  • Individualized Action Plans: Develop and implement personalized plans to help residents reach their objectives, promoting independence and self-sufficiency.
  • Resource Coordination: Connect residents with local resources and services, ensuring they have access to the support they need to thrive.
  • Group Facilitation: Lead groups on topics such as personal responsibility, social skills, and community living skills, promoting healthy relationships and behaviors.
  • Documentation and Reporting: Maintain accurate and thorough records of resident progress, ensuring compliance with regulatory requirements.
  • Interdisciplinary Collaboration: Share information and best practices with other staff members, promoting a cohesive and supportive team environment.
Requirements
  • Education and Experience: Bachelor's degree in a related field and a minimum of two years' experience in mental health, substance use treatment, and/or co-occurring disorders.
  • Certifications and Licenses: CADC may be substituted in place of a Bachelor's degree.
  • Skills and Qualities: Strong written and verbal communication skills, knowledge of milieu therapy, substance use modalities, harm reduction approaches, trauma-informed care, and whole-person health approaches.
  • Computer Proficiency: Proficient in electronic health records and Microsoft Office applications.
