Wraparound Facilitator

Cummins Behavioral Health Systems is seeking a dedicated and compassionate Wraparound Facilitator to join our team. As a Wraparound Facilitator, you will play a vital role in providing hybrid services to children and families in Hendricks County, Indiana.

The Wraparound Facilitator will work closely with children, families, and community professionals to develop and implement individualized plans that promote strengths-based, family-centered care. This role requires strong communication and interpersonal skills, as well as the ability to creatively access community resources.

• Advocate for children and families by engaging community volunteers and professionals in a strengths-based, family-centered planning process
• Develop and implement individualized plans that promote strengths-based, family-centered care
• Collaborate with community professionals to provide wraparound services
• Provide support and guidance to children and families in achieving their goals

• Bachelor's degree in social work, psychology, or a human services related field
• Two to three years of experience working with children in the mental health field
