Job Summary

CABVI is seeking a highly skilled Occupational Therapist Assistant/Care Coordinator II to join our team. As a key member of our rehabilitation services, you will provide occupational therapy to children and adults, enabling them to achieve their highest level of functioning.

Key Responsibilities
  • Provide occupational therapy to adults and children, including personal life management, communication, home management, and leisure activities under the supervision and clinical direction of an Occupational Therapist.
  • Assist Occupational Therapist in development of occupational therapy protocols, procedures, interventions, and policies, updating as needed.
  • Meet contractual and grant obligations, as well as established occupational therapy clinical productivity goals.
  • Provide vision therapy training to children identified through screenings and direct referral.
  • Respond to community inquiries regarding services for previously unserved consumers, providing comprehensive and accurate information on all therapy and programs available.
  • Complete regular and ongoing follow-up with consumers who have completed services, assessing if additional services are needed and re-opening those consumers for whom circumstances have changed.
  • Determine eligibility for funded programs, advocating with funding program staff as needed, and conveying financial responsibility to the consumer.
  • Develop rehabilitation plans for both new and reopened consumers, making internal therapy referrals and monitoring timeliness of services, general progress, and documenting therapy provided.
  • Assist consumers in applying for programs available to those with visual impairment, such as the NY State Park Pass, NY State Veterans Annuity Benefit, Talking Books, and others.
  • Make internal referrals to agency departments or services, including social work, orientation and mobility, vision rehabilitation therapy, occupational therapy, low vision, assistive technology, and more.
  • Make referrals to community programs when needs are identified, such as childcare, food bank, HEAP applications, and more.
  • Distribute marketing materials to community groups for promotion of special events, reaching out to consumers to solicit special event program engagement, and providing representation at community events to promote services.
  • Follow up on equipment dispensed to patients who receive low vision exams at one of our low vision centers, ensuring devices are used successfully, reviewing device care and usage, identifying natural supports, and working toward implementing assistance.
  • Provide VRT/OT instruction, under the supervision of a VRT, in areas of communication, dispensing adaptive equipment as needed, and assisting VRT's/OT staff with group instruction.
  • Consult, refer, and network with primary caregivers, NYSCB personnel, associated professionals, and CABVI staff.
  • Maintain awareness of current academic material and research, continuing professional development in the rehabilitation field, and completing any vision rehab identified research.
  • Develop a weekly schedule to maximize efficiency, entering your schedule on Outlook, documenting mileage and services on TICR daily, and completing all required documentation in a timely manner.
Requirements
  • Minimum of an Associate's degree and COTA certification required.
  • Experience with adaptive sports and recreation, serving individuals with vision loss, and children's recreational programming a plus.
  • Experience with community presentations, public speaking, or sales helpful.
  • Ability to effectively present information and respond to questions from consumers, professionals, and the general public.
  • Ability to write effective plans, reports, and correspondence.
  • Ability to provide visual cues according to consumer's visual status.
  • Excellent listening skills and ability to develop confidence and self-esteem.
  • Basic computer skills to effectively use Microsoft Word, Excel, CIS, and TICR, as well as knowledge and current awareness of low vision aids and adaptive equipment.
  • Occasional lifting and/or moving up to 50 pounds, with stamina for indoor/outdoor training.
  • Extensive travel required, with flexibility to meet business demands and alter work schedule/work hours as needed.
