Patient Care Advocate

We are seeking a highly motivated and compassionate Patient Care Advocate to join our healthcare organization. Located in Chicago, Illinois, this role is eligible for a hybrid remote work arrangement.
The estimated salary range for this position is $60,000 - $80,000 per year, based on industry standards and the level of experience required.
About the Role:
This role requires a strong understanding of healthcare systems and processes. The successful candidate will serve as a vital link between patients and our healthcare team, guiding them through care plans, ensuring follow-up, and promoting healthy lifestyles.
Key Responsibilities:
- Act as peer support for enrolled patients, advocating on their behalf as they navigate the medical system and building relationships with individuals and their families.
- Manage inpatient and outpatient utilization and medical care follow-up, ensuring seamless coordination of care.
- Perform population-based management activities, identifying health risks and developing strategies to mitigate them.
- Ensure timely completion of health risk assessments and provide relevant information to stakeholders.
- Maintain accurate records of patient interactions and outcomes.
Requirements:
- Associate's degree or 3+ years of health-related experience; Bachelor's degree preferred.
- Proficiency in Electronic Health Record systems, MHN Connect, Microsoft Word, and Excel.
- A minimum of 3+ years of experience in healthcare or a related field.
