Spanish-speaking Physical Therapist for Home Health

FeldCare Connects is a pioneering app-based referral network that is transforming the way home health care is delivered. We are seeking a highly motivated and skilled Spanish-speaking Physical Therapist to join our network and provide quality care to patients in their homes.
About the RoleAs an independent contractor, you will have the flexibility to control your own schedule and work in your preferred coverage areas. You will be responsible for delivering one-on-one treatment to patients, observing and recording their response to treatment, and reporting changes to their condition. Our team provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance, so you can focus on what you do best.
Key Responsibilities- Deliver high-quality physical therapy services to patients in their homes
- Develop and implement individualized treatment plans
- Communicate effectively with patients, families, and healthcare teams
- Document patient progress and outcomes
- Participate in quality assurance and improvement initiatives
- Current Physical Therapist license issued by the state Board of Physical Therapy
- Completion of an accredited Physical Therapy program
- Reliable personal transportation
- Smartphone
- Bilingual a plus
