Key Responsibilities:
- Develops and oversees patient care strategies. Observes, documents, and communicates patient status as necessary.
- Acts as the primary liaison among various disciplines to ensure seamless patient care.
- Executes physician and nursing directives.
- Evaluates and coordinates the discharge planning requirements of patients with the healthcare team.
- Utilizes the current body of evidence-based practices and scientific knowledge in healthcare to inform nursing practices, ensuring that care is tailored to the patient's age and clinical requirements as outlined in the department's Scope of Service.
- As a nurse at SSM Health, I will uphold the professional nursing standards defined in the practice model. I will apply the ANA Code of Ethics for Nurses to navigate the evolving health and nursing needs of our patients and communities.
- Maintains a constant state of vigilance and operates in a safe manner.
- Performs additional tasks as assigned.
Educational Qualifications:
- Graduate from an accredited nursing program or equivalent educational background for licensure.
Experience Requirements:
- Minimum of one year of experience as a registered nurse.
Physical Demands:
- Continuous verbal communication to convey information.
- Regular standing and walking.
- Frequent lifting, carrying, and pushing/pulling items weighing up to 25 lbs.
- Regular sitting, reaching, and data entry.
- Frequent visual acuity for both near and far distances, including color recognition.
- Regular olfactory recognition for detecting odors.
- Frequent auditory skills to receive verbal communication and recognize alarms or malfunctions.
- Occasional lifting and moving of items weighing between 25-50 lbs.
- Occasional patient handling.
- Occasional bending, stooping, kneeling, and other physical movements.
