Job Summary

The Home Health Social Worker is a vital member of our interdisciplinary team, providing essential services to patients and their families to improve, restore, and maintain their maximum level of coping. This role requires a strong understanding of community resources and the ability to collaborate with healthcare professionals to deliver high-quality care.

Key Responsibilities

• Communicate identified needs and potential solutions to the clinical team and supervisor.
• Function as an interdisciplinary team member, attending and participating in staff meetings, educational programs, and community events as requested.
• Complete psychosocial assessments on assigned patients, documenting findings in a timely manner.
• Develop and update care plans to meet the unique needs of each patient.
• Visit patients as indicated, providing direct social work services and monitoring hospitalizations and Medicare Part A admissions.
• Obtain physician orders for Home Health Social Services and facilitate placements and monitor patient adjustment.
• Coordinate caregiving issues and facilitate discharge planning, explaining and assisting with advance directives as needed.
• Address financial concerns and complete financial assessments, contacting community agencies as appropriate.
• Initiate Medicaid spend down and assess appropriateness for Special Concern supplies, evaluating insurance concerns and developing and utilizing community resources.
• Supervise social work practicum students and develop and maintain working relationships with community agencies, including long-term care facilities, DFS, hospitals, social security offices, V.A., and other relevant organizations.
• Utilize available community resources to meet family needs, working in cooperation with community agencies and lay groups to provide education and in-services as needed.
• Provide counseling to patients and families, addressing psychosocial needs and collaborating with the home health interdisciplinary team on counseling issues.
• Provide crisis intervention to patients and families, referring to community counseling agencies as indicated and offering and facilitating memorial services as directed.

Requirements

• Bachelor of Science in Social Work required, or Master of Science in Social Work degree preferred, unless otherwise stated by state-specific requirements.
• At least one year of social work experience in a healthcare setting required, with a Bachelor's degree in Psychology, Sociology, or other field related to social work and one year of social work experience in a healthcare setting and supervised by an MSW considered.
