Job Summary

Akahi Associates, LLC is seeking a highly skilled RN Case Manager to join our team in providing exceptional healthcare services to our clients. As a RN Case Manager, you will be responsible for participating in all phases of the Case Management Program (CMP) and ensuring that the CMP meets established case management standards of care.

Key Responsibilities
  • Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.
  • Develop and implement local strategies using inpatient, outpatient, onsite, and telephonic case management.
  • Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, case management software, virtual health modalities, and databases for community resources.
  • Maintain liaison with appropriate community agencies and organizations.
  • Accurately collect and document patient care data.
  • Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
  • Establish mechanisms to ensure proper implementation of patient treatment plans and follow-up post-discharge in ambulatory and community healthcare settings.
  • Provide appropriate healthcare instruction to patients and/or caregivers based on identified learning needs.
  • Provide transgender education related to clinical and/or case management nursing expertise to healthcare professionals as needed.
Requirements
  • Bachelor's of Science in Nursing and graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
  • Certifications in addition to Basic Life Support: Possess one of the following certifications: Commission for Case Manager Certification Certified Case Manager (CCM), Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS), Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN), American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S), National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC), Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC), American Nurses Credentialing Center Nurse Case Manager (RN-NCM), National Academy of Certified Care Managers: Care Manager Certified (CMC).
  • A minimum of 2 years as a RN with full-time experience in utilization management/utilization review or in broad-based clinical nursing and clinical case management working with adults, children, families, seniors, and groups within the last 3 years.
  • Have completed 30 hours of continuing education in nursing within the last 3 years.
  • Current, full, active, and unrestricted license as a Registered Nurse.
  • Must attend continuing education annually to ensure continued awareness and expertise in clinical advances and best practices for gender-affirming healthcare.
