Job Title: Case Manager

Interfaith Works is seeking a skilled Case Manager to join our Homeless Services team. As a Case Manager, you will play a critical role in supporting individuals and families experiencing homelessness to achieve stable housing and improve their overall well-being.

Key Responsibilities:
  • Work collaboratively with the Program Director and multidisciplinary team to provide trauma-informed support to residents.
  • Build rapport with clients and assess their ongoing needs to make referrals for services that promote housing stability.
  • Provide a range of client services, including referrals, life skills training, health maintenance, and transportation to essential appointments.
  • Update client charts and files regularly, documenting all client communications in our Homeless Management Information System (HMIS).
  • Meet with apartment managers and landlords to ensure client stability and negotiate new leases annually.
  • Conduct home inspections monthly and maintain communication with Montgomery County, landlords, and building management companies.
  • Attend meetings with the County, including monthly teaming meetings and Housing Prioritization meetings.
  • Participate in Interfaith Works meetings, including Case Management & Vocational meetings, and other activities.
Requirements:
  • Bachelor's degree in a social services-related field; Master's degree in Social Work, Counseling, or Psychology preferred.
  • Two years of direct service experience working with individuals who are homeless and have disabilities.
  • Understanding of trauma-informed care and the Housing First model.
  • Experience with crisis intervention, including assessing for and responding to individuals with suicidal and homicidal ideation.
  • Ability to manage multiple responsibilities efficiently and work independently and collaboratively as part of a team.
  • Excellent written and oral communication skills and proficiency with Microsoft Windows and Office365.

The noise level in the work environment is usually low to moderate.
