**Job Summary:**
We are seeking a highly skilled Advanced Practice Nurse to join our team at Abode Care Partners. As a key member of our medical staff, you will provide personalized medical care to seniors and patients with complex needs.
Key Responsibilities:
- Medical Care Planning: Develop and implement comprehensive care plans to meet the unique needs of our patients.
- Patient Assessment and Treatment: Conduct thorough assessments, diagnose, and treat chronic and urgent episodic illnesses, minor injuries, and other medical conditions.
- Collaboration and Communication: Work closely with caregivers, healthcare professionals, and other stakeholders to ensure seamless care coordination and effective communication.
- Medical Record Management: Maintain accurate and up-to-date patient medical records, ensuring compliance with regulatory requirements.
- Referrals and Consultations: Provide referrals to other healthcare practitioners as needed and collaborate with specialists to ensure comprehensive care.
Requirements:
- Education and Licensure: Master's degree in Nursing (MSN) and active state Nurse Practitioners license.
- Experience: Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
- Skills: Strong interpersonal communication skills, excellent time management skills, proficiency in medical instruments and equipment, and knowledge of HIPAA and OSHA regulations.
- Personal Qualities: Ability to engage in moderate physical activity, strong knowledge base, and demonstrated proficiency in computer-based data management programs.
