Job Title: Social Worker

Osborne Association is seeking a dedicated Social Worker to join our team. As a Social Worker, you will work directly with clients, engaging with them, assessing their needs, developing service plans, making referrals, providing advocacy, counseling, reassessment, and summarizing their exit.

Key Responsibilities:
  • Manage a caseload of high needs clients population at Rikers Island
  • Collaborate with child welfare agencies to ensure visiting is being held as ordered
  • Conduct one-on-one Therapeutic counseling sessions as needed to focus on individual parenting, discharge planning and service needs
  • Maintain both group and individual records and case notes as needed
  • Provide referrals for incarcerated individuals to promote successful community re-entry
  • Provide referrals to family members in the community to support their involvement in the criminal justice system
  • Document all individual and group work in DOC and Osborne Association databases, including but not limited to a DOC tracker, Salesforce/ECM and others as needed
  • Coordinate with DOC and MOCJ providers at RMSC to avoid duplication in services and ensure that participants have all of the support they need to successfully reunite with their families
  • Escalate and collaborate with the Program Coordinator of any complex individual or family needs that may arise to provide resolution in a timely manner
  • Participate in all relevant program and CYFS meetings
  • This position will occasionally be required to work weekend and/or evening hours
  • Assist with individual/group therapeutic services during incarceration and upon release to the community
  • Monitor and document client progress toward service plan goals; document all interactions with, or on behalf of, clients
  • Coordinate client services with mental health, medical, New York City Department of Correction staff, New York City Board of Education School staff and other social service providers at Rikers Island
  • Communicate and collaborate with client support team and service providers to enhance engagement and ensure quality of service
  • Address clients concerns and goals while maintaining constant communications with the clients as well as a case manager
Requirements:
  • Master's degree in social work required
  • One year of relevant work experience is preferred
